The Ultimate Checklist for the Last Trimester of Pregnancy

Reaching the last trimester of pregnancy is like seeing the sunrise after a long night: filled with excitement, anticipation... and also many questions.

From here, between week 28 and birth, your body and your baby are putting the final touches on getting ready to meet each other. I want to support you through this stage with a loving and practical checklist so that each step feels more secure and connected to you and your baby.

  1. Keep up with your prenatal appointments
    Your healthcare team is your main ally in these weeks. During this trimester, visits are usually every 2 to 4 weeks. These checkups monitor your blood pressure, the baby’s growth and position, and ensure everything is progressing well. They may also check fetal movements and the heartbeat to make sure your baby is healthy.
    Mom tip: Bring a list of questions or symptoms you want to discuss at each visit. There’s no detail that’s “too small” if it helps you feel more at ease.
  2. Monitor the baby’s movements
    One of the most beautiful ways to connect with your baby during these weeks is feeling their movements inside you. Movement patterns are also a helpful tool for your health: reporting fewer movements than expected can alert you to potential concerns and help you receive timely care.
    My advice: When you lie down or relax, gently place your hand on your belly and celebrate every little kick — they’re preparing for that first hug!
  3. Start your birth plan and prenatal classes
    This trimester is a great time to think about and talk through your birth plan with your medical team, and if possible, take classes on childbirth, breastfeeding, or newborn care. These experiences not only provide you with practical information but also build your emotional confidence for the big day.
    Preparing doesn’t mean controlling everything; it means being equipped with knowledge and the peace of mind that you’re ready for whatever path your birth journey may take.
  4. Prepare your space and essentials for the baby
    Your “nest” doesn’t have to be perfect: it just needs to be safe and welcoming.
    Make sure the place where your baby will sleep meets current safety standards — such as a crib or bassinet with a firm mattress and fitted sheets, without pillows or toys — and that you have diapers, wipes, and a changing station ready.
    You don’t need millions of things. In the last trimester, what matters is having the basics organized and feeling like you’re creating a warm environment for both your baby and yourself.
  5. Take care of your physical and emotional well-being
    It’s normal to experience heartburn, fatigue, or difficulty sleeping as your body adjusts to the baby’s growth. Listening to your body is essential: rest, take gentle walks if it feels good, stay hydrated, and follow your doctor’s advice about exercise and nutrition.
    Your well-being is part of your baby’s well-being. Don’t hesitate to ask for help, share how you’re feeling, and lean on your loved ones.
  6. Signs you shouldn’t ignore
    While every pregnancy is different, there are changes that require immediate medical attention: heavy bleeding, sudden swelling, marked decrease in baby’s movement, or headaches with vision changes. Reporting any unexpected changes can make a big difference for both you and your baby.

This trimester is a time of preparation, connection, and self-care. You’re not alone — each appointment, each kick, each emotional adjustment is part of your story as you get closer to meeting your baby. Breathe, trust your body, and remember that you’re doing something wonderful.
